Internal Medicine Referral Form Test
What level of urgency applies to this patient?
Level of urgency
*EMERGENCY*
– Immediate action needed (e.g. AKI, urinary obstruction, severe anemia, respiratory distress, hyperbilirubinemia, diabetic ketosis, severe or symptomatic hypercalcemia, cats with complete anorexia > 2 days, acute onset severe vomiting/diarrhea, acute onset severe lethargy or inability to walk, addisonian crisis).
STOP! Please call the NC State Emergency Service 919-513-6911 instead of completing online referral.
Level of urgency
Level 1 (Urgent) – Should be seen within 6-8 weeks (e.g. effusions, dysphagia or megaesophagus, epistaxis, melena or hematochezia, moderate-to-severe hypoalbuminemia, symptomatic liver enzyme elevations, fever of unknown origin, thoracic or abdominal masses*, rapid weight loss, hyporexic cats) *You may find a traveling ultrasonographer can perform an ultrasound with needle aspirates more timely than referral
Level 2 (Semi-Urgent) – Should be seen within 8-10 weeks (e.g. chronic vomiting, chronic cough including suspected collapsing trachea, recurrent pneumonia, asymptomatic hypercalcemia, mild hypoalbuminemia)
Level 3 (Non-Urgent) – Can wait 10+ weeks (e.g. PU/PD, diabetes (not DKA), asymptomatic liver enzyme elevations, chronic rhinitis, chronic diarrhea)
Level 4 – Second Opinion from a Specialist – If this referral is for a second opinion on a case that is already being managed by another veterinary specialist (e.g., another internist, oncologist, neurologist, etc.). 📌 Important Note: Due to limited availability and appointment capacity within our Internal Medicine Service, we will only accept second opinion referrals initiated directly by the managing specialist, rather than the primary care veterinarian. This ensures continuity of care, clarity of case status, and optimal communication between specialists.
If you are requesting a Level 1 (Urgent) or Level 2 (Semi-Urgent), please briefly explain the rationale for this urgency. This allows us to prioritize patients and ensure timely care. For example, include any recent clinical deterioration, concerning diagnostics, or limitations in outpatient management.
Has this patient been seen at a specialty hospital?
(Required)
Yes
No
Please contact the hospital to have their records forwarded to us.
Due to overwhelming demand for specialty care limiting availability for new patient appointments, if the patient has been seen by another internist we are not scheduling for an owner requested second opinion. We will accept a second opinion referral directly from the other internist, however.
